Efectos de la terapia láser y la maniobra de acortamiento muscular de Grimaldi en el control motor de sujetos con lesiones incompletas de la médula espinal

Diego Longo, PT, Leonardo Longo, MD, Paolo Lippi, PT, Giulio Cherubini, PT, Vanessa Mange, PT, Instituto de Medicina Láser, Florencia, Italia Antecedentes: Desde 2003 hasta la actualidad hemos tratado a 251 pacientes con lesiones traumáticas del sistema nervioso central (TCNSI), utilizando terapia láser no quirúrgica (NSLT) obteniendo buenos resultados en cuanto a sensibilidad y movimiento. Para aumentar la fuerza muscular y explorar más a fondo las nuevas sinergias emergentes, hemos comenzado a utilizar una práctica de fisioterapia basada en los conocimientos más actuales sobre el control motor, denominada maniobra de acortamiento muscular (MSM) de Grimaldi. Cada TCNSI es diferente; no existen dos semejantes en cuanto a la pérdida de función y respuesta al tratamiento. Por esta razón, el criterio estadístico es relativo, ya que hay demasiadas variables implicadas al mismo tiempo. El objetivo de nuestro estudio es obtener datos objetivos que sugieran la eficacia real de la asociación entre estos dos tratamientos. Estudio: En 2015, se inscribieron 10 pacientes con TCNSI incompleto, ocurrido al menos un año antes del tratamiento con láser y documentado por NMR, ESSP y ESMP. Al mismo tiempo, se incluyeron como grupo de control otros 10 pacientes con las mismas características. Todos los pacientes tienen parálisis sensorial y motora subtotal por debajo del nivel de la lesión. Los láseres utilizados fueron de 808 nm, 10600 nm y 1064 nm, aplicados con un primer ciclo de 20 sesiones, cuatro veces al día. Los pacientes participaron en un entrenamiento específico de fisioterapia para MSM de Grimaldi dos veces al día, ocho sesiones en total, trabajando selectivamente en ciertas articulaciones, músculos y extremidades. Antes del tratamiento por debajo del nivel de la lesión, se evaluó la actividad muscular con el sistema EMG de superficie (sEMG). Las evaluaciones clínicas incluyeron la investigación de la sensibilidad táctil y térmica superficial y profunda bajo el nivel de lesión. Se evaluó la fuerza muscular en ángulos articulares específicos mediante dinamómetros y goniómetros electrónicos. Este último tipo de evaluación también se llevó a cabo antes y después de cada MSM y al final del ciclo del tratamiento con láser. Cada ciclo de ambos tratamientos se repitió cada mes. Los controles no recibieron ningún tipo de tratamiento, pero se evaluaron todas las variables para el grupo de intervención. Resultados: Los resultados se consideraron positivos si el sEMG mostraba modificaciones en los picos de conducción entre el músculo y el sistema nervioso central, por debajo del nivel de la lesión. La evaluación objetiva de la fuerza, la amplitud de movimiento del tronco y el equilibrio mostró resultados alentadores en los pacientes asignados al grupo de intervención. En el TSCI subtotal, después de cada ciclo de 20 sesiones, los pacientes mostraron mejoras en la función motora y el comando voluntario mostrado por las características gráficas. El seguimiento es positivo a los 3 meses. Conclusión: La asociación entre el tratamiento con láser y la maniobra de acortamiento muscular (MSM) de Grimaldi parece ser eficaz en la fuerza muscular y el control motor de los pacientes afectados por SCI subtotal en comparación con un grupo de control.

About the Role:

We’re looking for an enthusiastic, creative and detail-oriented problem solver to help support the production, service and repair of our unique solid-state laser devices.

Must have expertise working with assembly and/or development of small, sensitive, high precision instruments, preferably solid-state laser technology, with a high caliber technology company.  We prefer design and hands-on experience in multiple laser technologies including solid state lasers, semiconductor lasers, non-linear optical materials and devices, optical benches, and beam directors. Experience with the technical aspects of developing laser and laser-based products is strongly preferred.

Candidates should have proven ability to address complex challenges with ingenuity and innovation, and capability to work independently to manage multiple simultaneous tasks, and see all tasks to completion. Also should have the ability to work well in a collaborative multidisciplinary team environment, define and analyze system performance specifications, and ensure adherence to those specifications in the application environment.

  • Perform sensitive manual optical assembly of lasers.
  • Test, troubleshoot repair and assemble lasers, subassemblies and various components, including high-voltage laser power supplies and other electronics.
  • Support the development and implementation of test fixtures, equipment, procedures and documentation for the engineering and operations functions.
  • Support product design and development activities related to new products and product improvements.
  • Support development and maintenance of quality and regulatory documents and procedures.
  • Provide customer technical support as needed including product service, maintenance, and troubleshooting.
  • With suppliers, act as a technical and operational liaison as needed.
  • Maintain thorough, complete and accurate written records; comply fully with the company’s quality system and any regulatory requirements.
  • Follow safety rules for working with high-voltages and high-power lasers.
