Tratamiento de las telangiectasias de las piernas con un láser Nd:YAG pulsado de 650 microsegundos de 1064 nm

Los láseres Nd:Yag con anchos de pulso de 20 a 30 milisegundos se utilizan tradicionalmente para tratar las venas cosméticas de las piernas, con efectos secundarios bien documentados de hematomas y malestar, especialmente en áreas grandes tratadas. Este estudio se llevó a cabo para evaluar la seguridad y la eficacia clínica de un nuevo láser Nd:Yag de pulsos cortos y 650 microsegundos de 1064 nm para el tratamiento de las telangiectasias de las extremidades inferiores. Diseño y métodos del estudio: Se seleccionaron mujeres adultas con tipos de piel I-III en la escala de Fitzpatrick con telangiectasias en las piernas de 0.2 a 3 mm de diámetro que cubrían al menos 20 cm2 para realizar sesiones individuales de tratamiento con láser a intervalos de 4 semanas hasta que se observó una eliminación clínica satisfactoria. Los parámetros del tratamiento con láser variaron según el tamaño, la cantidad y la densidad de los vasos de la siguiente manera:
Vasos con menos de 1 mm de diámetro – Tamaño de punto de 2 mm, Fluencia 127 J/cm2, ancho de pulso de 650 microsegundos
Vasos con diámetro de 1 a 3 mm – Tamaño de punto de 2 a 3 mm, Fluencia 159-223 J/cm2, ancho de pulso de 650 microsegundos. Se utilizó una compresa de hielo para el preenfriamiento durante 20 a 30 segundos antes del láser.
Los vasos se trataron en toda su longitud y se observaron para determinar su eliminación. Se monitorearon los eventos adversos de malestar, ampollas, costras, cicatrices y duración del eritema durante las primeras 4 semanas después del tratamiento. Se tomaron fotos antes e inmediatamente después del tratamiento y a las 4 y 12 semanas después del tratamiento. El seguimiento continuó durante 12 meses para evaluar la recurrencia. Resultados: Se trataron 22 mujeres en las extremidades inferiores. El 72 % de los sujetos mostró una mejora considerable a las 4 semanas después del primer tratamiento. El 96 % de los pacientes mostró una mejora considerable 3 meses después del tercer tratamiento. En todos los sujetos apareció un eritema leve inmediatamente después del tratamiento y duró de 3 a 24 horas. Los vasos pequeños desaparecían con frecuencia durante el tratamiento, mientras que los vasos más grandes de más de 1 mm cambiaban de color oscureciéndose. 5 pacientes exhibieron costras de color marrón claro a lo largo del vaso coagulado que no dejaron marcas permanentes en la cita de seguimiento de 3 meses. No se observaron quemaduras, cicatrices o hiperpigmentación en ninguno de los pacientes. El dolor se calificó como leve durante las sesiones de tratamiento. Conclusión: Las telangiectasias de las extremidades inferiores que oscilan entre 0.1 y 3 mm se pueden tratar de forma segura y eficaz en los tipos de piel I-III con un nuevo láser Nd:Yag pulsado de 650 microsegundos con una incomodidad mínima.
