Lip Rejuvenation and Lip Plumping with 650-microsecond Pulsed Nd:YAG 1064nm Laser (Lippenverjüngung und Lippenaufpolsterung mit einem Nd:YAG-Laser mit einer Pulsdauer von 650 μs und einer Wellenläge von 1064 nm)

Hintergrund und Zielseetzung: Traditionell erfolgt die Augmentierung von Lippengröße und -form mit Hyaluron-Injektionen. Medizinische Spas im Besitz und unter der Leitung von medizinischen Kosmetologinnen stehen vor dem Problem, dass sie aufgrund bundesstaatlicher Vorschriften bei ihren Patienten keine Füllstoffe injizieren dürfen.

Lichtbasierte Laserbehandlungen der Lippen können eine wesentliche Aufpolsterung sowie Farb- und Texturverbesserung und eine signifikante Reduzierung feiner Linien und Falten um die Lippen herum bewirken. Bei diesem Verfahren wird das Lippengewebe und der umgebende Bereich erwärmt, um Kollagen zu bilden. Patienten mit Nadelphobie oder Angst vor Schmerzen oder Blutergüssen bei Nadeleingriffen können von einer nadelfreien Lippenbehandlung profitieren. Der Laser muss jedoch in der Lage sein, Energie tief in das Hautgewebe abzugeben, um die Kollagenese bei einer verträglichen Patientenerfahrung zu ermöglichen. Der Zweck der Studie bestand darin, die Wirksamkeit der Behandlung zur Lippenverjüngung und Lippenvergrößerung mit einem Nd:YAG-Laser mit einer Pulsdauer von 650 μs und einer Wellenläge von 1064 nm zu bewerten.

Materialien und Methoden: Es wurden 10 Studienteilnehmer im Alter von 18 bis 65 Jahren mit Fitzpatrick-Hauttypen I bis IV und mit mittelstarken bis starken Lippenlinien oder Fältchen ausgewählt. Die Studienteilnehmer wünschten keine Hyaluron-Füllstoffe per Injektion, sondern eine alternative Behandlung.

Den Studienteilnehmern dieser Studie wurde vor oder während der Studie kein Hyaluron-Füllstoff injiziert. Vor der Behandlung wurde die Haut mit einem Gesichtsreiniger gereinigt. Während der gesamten Studie wurden Fotos vor und nach den Behandlung aufgenommen. Zur Behandlung wurde ein Nd:YAG-Laser mit einer Pulsdauer von 650 μs und einer Wellenlänge von 1064 nm (Neo-Laser, Aerolase Corporation, Tarrytown NY) verwendet.

Jeder Studienteilnehmer unterzog sich sechs bis acht Behandlungen im Abstand von ein bis zwei Wochen. Die Studienteilnehmer wurden mit Fluenzen im Bereich von 25 bis 41 J/cm2 behandelt. Vor oder während der Behandlungen wurden keine Anesthetika verwendet. Laserpulse wurden in einem Kreuzschraffurmuster auf den Lippenbereich aufgebracht; das Muster bedeckte die gesamte Lippenbasis sowie die umgebenden Lippenbereiche mit feinen Linien. Die Behandlung umfasste acht bis zehn Durchgänge über den gesamten Behandlungsbereich, wobei eine Spotgröße von 6 mm mit einer Wiederholungsrate von 1,25 bis 1,5 Hz verwendet wurde und die Wiederholungsrate von der Toleranz des Studienteilnehmers abhängig war. Die Behandlung wurde in einigen Fällen mit einer 5-mm-Linse und Fluenzen im Bereich von 25 bis 41 J/cm2 fortgesetzt, wobei drei Durchgänge über den behandelten Bereich durchgeführt wurden. Die Studienteilnehmer wurden jeweils 6 bis 8 Mal in Abständen von 1 bis 2 Wochen behandelt. Die Studienteilnehmer wurden sowohl während der Behandlung als auch unmittelbar danach zum Behandlungskomfort und zur Schmerztoleranz befragt.

Ergebnisse: Die Studienteilnehmer verspürten während der Durchgänge sieben und acht leichtes Unbehagen. Dieses Unbehagen war bei älteren Studienteilnehmer (Altersgruppe 25 bis 70) nicht untolerierbar. Die Studienteilnehmer berichteten nicht von Nebenwirkungen nach der Behandlung, und es wurden keine Anzeichen von Herpesausbrüchen beobachtet. Unmittelbar nach jeder Behandlung verspürten die Studienteilnehmer einen Rouging-Effekt (heller in verschiedenen Rottönen), eine merkliche Augmentierung der Lippen und ein kurzzeitiges Erythem nach der Behandlung, das innerhalb weniger Stunden abklang. Die Ergebnisse wurden ohne die für Lippeninjektionen typischen Nebenwirkungen wie Klumpenbildung, Blutergüsse oder asymmetrische Ergebnisse erzielt. Alle Studienteilnehmer berichteten am Tag nach der Behandlung von wesentlichen Vergrößerungen. Bei der dritten Behandlungssitzung berichteten die Studienteilnehmer, dass feine Linien und Fältchen um den Lippenbereich signifikant auf ein von jedem Studienteilnehmer bestimmtes Zufriedenheitsniveau reduziert wurden.

Am Ende von sechs bis acht Behandlungen berichteten die Studienteilnehmer von einer signifikanten Verbesserung der Farbe, Textur und dem jugendlichen Aussehen ihrer Lippen. Die Studienteilnehmer berichteten auch, dass die Verbesserung drei bis sechs Monate anhielt, wobei jüngere Studienteilnehmer eine Wirkung von bis zu sechs Monaten beobachteten. Studienteilnehmer ab 60 Jahren gaben an, dass die Verbesserung bis zu drei Monate anhielt. Die Studienteilnehmer berichteten, dass sie die Behandlung bei Bedarf in Zukunft fortsetzen und anderen die Behandlung empfehlen würden.

Schlussfolgerung: Die Lippenverjüngung und -aufpolsterung mit einem Nd:YAG-Laser mit einer Pulsdauer von 650 μs und einer Wellenlänge von 1064 nm kann ohne den Einsatz von Nadeln, ohne Nebenwirkungen und mit minimalen Beschwerden durchgeführt werden. Das Verfahren sorgt für hohe Patientenzufriedenheit und weckt das Interesse an weiteren Behandlungen.

  • Perform sensitive manual optical assembly of lasers.
